Privacy of children
We do not knowingly collect any Personal Information from children under the age of 13. If you are under the age of 13, please do not submit any Personal Information through our Website or Service. Overview of Pardigm services
Pardigm provides a platform to facilitate a consultation between affiliated Providers and you. Pardigm does not provide medical services, including via the Sites or Services.
Pardigm also provides software resources for you to optimize your wellness through measuring cortisol levels. Personally identifiable information you share with Pardigm in the course of using features related to this area will only be used by Pardigm and its affiliates in manners described below to enable your use of the Service. You maintain control of your data and can view or opt-out of sharing data with Pardigm, although in doing so, some features may stop working for you.
Disclaimer of HIPAA Applicability.
Pardigm is not a “covered entity” as defined in the federal medical record privacy statute known as the Health Insurance Portability and Accountability Act of 1996, as amended, and the regulations issued under that law (“HIPAA”). Additionally, with respect to the personal information that Pardigm will obtain about you, Pardigm does not anticipate that it will ever be a “business associate” under HIPAA, since the companies Pardigm works with are not “covered entities” under HIPAA. Therefore, even though your personal information contains medical information that is the type of information commonly protected under HIPAA, it is not subject to any of the protections available to you under HIPAA. HIPAA is inapplicable to Pardigm’s relationship with you.
Collection of personal information
We receive and store any information you knowingly provide to us when you create an account, publish content, make a purchase, fill any online forms on the Sites, or engage with Pardigm and its affiliates. When required this information may include your email address, name, phone number, address, credit card information, or other Personal Information. You can choose not to provide us with certain information, but then you may not be able to take advantage of some of the Sites’ features. Users who are uncertain about what information is mandatory are welcome to contact us.
Automatic collection of information
When you visit the Sites our servers automatically record information sent by your web browser or mobile application. This data may include information such as your device's IP address, browser type and version, operating system type and version, language preferences or the webpage you were visiting before you came to our Sites, pages of our Sites that you visit, the time spent on those pages, information you search for on our Sites, access times and dates, and other statistics.
Use of collected information
